Plan Guidelines:
This Crescent Beach Smiles Membership Plan is NOT an insurance but will allow those who do not have insurance or those whose insurance won't cover all services, to take advantage of special savings. These dental benefits are specific to our practice and payment is always due at the time of service.
Membership dues are payable in one of the following methods: 1.) all up front 2.) 3 equal monthly, 3.) or 12 equal monthly payments.
Membership will automatically renew on the following year's date of purchase, unless opted out.
Payments for additional dental services are the member's responsibility. Payment is due on the date of service to qualify for the discounts detailed above.
Membership must be current to receive the discount.
Fees for dental services may change at any time.
It is the member's responsibility to schedule and keep all appointments offered as part of the Crescent Beach Smiles Plan.
Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
No deductions, pre-authorizations, yearly maximums, or waiting periods.
No exclusions for elective care.
